Health Choice Network, Inc. (HCN) is a national model of successful collaboration amongst health center controlled networks, community health centers and other health partners. Through its cutting-edge health information technology and business services, HCN participants increase efficiencies, decrease costs and provide better and more accessible patient care. HCN is a not-for-profit organization governed by a Board comprised of its members. HCN member centers and additional customers provide care to more than 2.2 million patients and play a vital role in improving the quality of life within the communities they serve.

OSIS is a non-profit technology services organization dedicated to providing expert NextGen Healthcare technology assistance exclusively to Community Health Centers around the country.

The Health Center Connections HCCN is organized and supported by the Community Care Network of Kansas, the federally designated Primary Care Association for Kansas.

Connections provides member health centers with a population health and analytics system, licenses to health IT tools, technical assistance, educational resources, training, and the opportunity to learn from and support each other through the successes and failures of individual experiences.

Community Care is a statewide association made up of a network of community care clinics striving to make sure all Kansans have access to high-quality, community-based, whole-person health care close to home.
